You are responsible for disclosing all insurance plans and coverages at the time of appointment. Realm Healthcare Inc. will bill primary and secondary insurance plans as a courtesy. However, the primary responsibility for the account is yours. If you have an HMO as a primary, please inform the office as soon as possible. Non-disclosure of all your insurance coverages can lead to non-payment for services provided and make you responsible for unpaid claims.

Some insurance plans have restrictions or a specific provider panel that patients must access. Please check with your insurance carrier for specific eligibility, benefits, provider panel and/or referral requirements as they apply to you.
